Assessment and herbal recording

Keep assessment processing and herbal dispensation evidence attached to the encounter.

Step by step

  1. Identify the assessment Licensed Clinician

Licensed Clinician records the assessment type and accession number against the encounter. Check the patient through the visit before starting collection. Use the agreed assessment naming convention so the receiving person can recognize the requested work without interpreting a personal abbreviation.

    • Identify requested work and visit
    • Updates Practitioner assessments, Encounters
  2. Record collection and processing Licensed Clinician

Licensed Clinician moves Ordered work to Collected when collection occurs and records the collection date. Confirm Received at the receiving step and Tested after processing. Use Rejected for an item the practice cannot accept, retaining its accession number so the clinical team can investigate the handoff.

    • Record the actual processing state
    • Updates Practitioner assessments
  3. Record herbal dispensation Licensed Clinician

Licensed Clinician records the preparation name and batch from the material dispensed, with quantity and unit. Identify the authorizing practitioner and the encounter. That practitioner owns the clinical decision; the entry gives the team an attributable record of what was dispensed.

    • Retain preparation, batch and authority
    • Updates Herbal dispensations, Encounters, People
  4. Prepare encounter review Licensed Clinician

Licensed Clinician checks the assessment state and dispensation entry alongside the clinical note. Resolve a mistaken encounter reference or unclear unit before Clinical Coordinator requests encounter review. Clinical Reviewer considers the clinical account through the encounter review process.

    • Assemble the clinical account for review
    • Updates Clinical notes, Practitioner assessments, Herbal dispensations

Where it usually breaks

And how to keep it from happening.

  • The accession number is copied onto an assessment for the wrong encounter.
  • Received is used when an item has only been collected, obscuring the handoff still ahead.
  • A rejected assessment disappears from the discussion instead of being addressed by the clinician.
  • Quantities with different units are combined because the preparation name is the same.
