Where it usually breaks

And how to keep it from happening.

  • A returning patient's new phone number leads staff to create a duplicate identity instead of checking the patient number.
  • A consent document is treated as current without inspecting its purpose or expiry date.
  • A signed note belongs to an earlier encounter, leaving today's reviewer without the intended account.
  • An encounter is completed while the reviewer is still waiting for clarification.
