Where it usually breaks

And how to keep it from happening.

  • A changed phone number prompts a duplicate patient entry. Check the existing patient number before continuing.
  • A note remains Draft when the encounter reaches review. The clinician resolves the documentation question before completion.
  • A reviewer decision is not passed back to the coordinator. Agree who communicates it and how the coordinator confirms receipt.
