Fix and Independently Verify

Assign the correction, hand it to QA and close only after the reported behavior has been independently checked.

Step by step

  1. Plan the correction Project Manager

Project Manager creates the corrective task within its work package, selects the assignee and sets priority. Include the issue key in the task description. Confirm that the task and work package belong to the intended project before discussing delivery dates or effort.

    • Assign corrective work and record its issue-key cross-reference
    • Updates Tasks, Work packages, Defect reports, Team members
  2. Perform the work Developer

Developer starts the correction and updates task progress. Record a blocking reason if work cannot continue. Keep the defect's owner informed and retain the original reporter notes so the expected behavior stays clear during investigation.

    • Advance active correction and explain blocked work
    • Updates Tasks, Defect reports
  3. Present the fix Developer

Developer marks the defect Fixed when the correction is ready and moves the task into review. Supply the context QA needs to repeat the reported behavior and locate the supporting evidence. Fixed signals readiness for assessment.

    • Present Fixed defect and task in review with evidence
    • Updates Defect reports, Tasks, Record attachments
  4. Verify independently QA Engineer

QA Engineer repeats the reported conditions and decides whether the correction is acceptable. Move the defect to Verified only after a successful independent check. A failed retest remains unverified, with an explanation for the Developer. Record acceptance of the corrective task using its actual task reference and project.

    • Authorize Fixed to Verified after independent assessment
    • Updates Defect reports, Review decisions, Record attachments, Tasks
ApprovalA person approves before the work moves on.
  5. Close the accepted work QA Engineer or Project Manager

QA Engineer checks that verification evidence is retained before closing the defect. Project Manager confirms the accepted task can become Done. The defect's verification and the task's completion must remain separately traceable.

    • Close verified defect and complete accepted task
    • Updates Defect reports, Tasks

Where it usually breaks

And how to keep it from happening.

  • A task description refers to the wrong issue key. Resolve the original report before starting the check.
  • The implementer accepts their own correction. Use an independent QA Engineer for verification.
  • A failed retest is marked Verified to clear the queue. Retain the failed result and continue corrective work.
  • A Done task is treated as proof that the defect was verified. Inspect the defect state and actual acceptance evidence.
