Report and Triage a Defect

Capture the reported behavior and use the severity and state checks to establish a triaged defect.

Step by step

  1. Record the report QA Engineer or Developer

QA Engineer or Developer records the summary and project, with the reporter notes explaining how to repeat the behavior. Keep the issue key available for later task cross-references. Identify the reporter and corrective owner where known, and start the report as Open.

    • Establish an Open defect with its reported context
    • Updates Defect reports, Projects, Team members
  2. Assess severity QA Engineer

QA Engineer chooses Critical, High, Medium or Low using the team's agreed definitions. Check that the notes explain the impact and expected result. Severity describes the defect; the Project Manager will separately decide the priority of corrective work.

    • Supply the severity required for triage
    • Updates Defect reports
  3. Run triage QA Engineer

The Defect Triage workflow processes the existing report when it is Open and severity is present. It moves the defect to Triaged and records an operation receipt. This is a state and completeness check, with no separate human approval gate.

    • Move Open to Triaged when the workflow guards pass
    • Updates Defect reports, Operation receipts
  4. Inspect the result QA Engineer

QA Engineer reads the receipt and source report. Missing severity or a report no longer Open causes a failed attempt. Correct the actual cause before retrying, and retain the earlier receipt so the team can explain what happened.

    • Explain success or failure against the source record
    • Updates Operation receipts, Defect reports
  5. Prepare the assignment QA Engineer

QA Engineer confirms the defect owner. Project Manager selects the work package, creates the corrective task and assigns its Developer. Include the issue key in the task description so the Developer can find the report and its reproduction context.

    • Confirm the defect owner
    • Updates Defect reports, Team members

Where it usually breaks

And how to keep it from happening.

  • Reporter notes do not explain the observed conditions. Ask for enough detail to repeat the behavior before planning the fix.
  • Severity is omitted. The triage attempt fails and the report still needs assessment.
  • An already triaged report is processed again. Inspect its current state and the failed-attempt receipt before retrying.
  • Task priority is treated as the defect's severity. Keep impact assessment and delivery sequencing distinct.
