Systems to connect

  • Patient communication. If reminders or follow-up messages come from another service, decide which scheduled visit or care task supplies the trigger and who reviews delivery.
  • Pharmacy. If signed prescription orders are sent externally, map the encounter, prescriber, medication and dose instruction to the receiving service and retain its acknowledgement.
  • Laboratory. If the clinic orders or receives lab work elsewhere, agree how staff identify the matching patient and encounter in the laboratory system.
  • Accounting and payments. Agree how encounter charges, patient payment references and confirmed amounts reconcile with the external financial source.

How connections are governed

Name the owner for each connection and the system that supplies the final answer. Check patient identity and encounter identity before exchanging clinical information. Test one successful handoff and one rejected or delayed response, then document how the team recovers. A local status should change only after the clinic has the evidence it expects from the external service.
