What you can do
Keep a named next action
A care task records the work, patient, assigned person, due time, completion time and current state. The Care Coordinator can use it to make a clinical follow-up visible after a visit. The assigned Clinician sees work that needs action, while the Clinical Lead can inspect overdue or escalated items. Tasks have Open, in progress, Completed, Escalated and Cancelled states. A task is still about a particular patient even when its work follows a completed encounter.
The clinic should decide when to create a follow-up during the visit handoff. The person creating it states a concrete task and due time rather than relying on a vague reminder. When the work is done, the assigned person marks it Completed and records the completion time. Escalated makes an exception visible to the coordinator and lead. Cancelled remains available when the task is withdrawn.
How the work is organized
- Patient. Each task identifies the patient whose care needs follow-up.
- Owner. The assigned person receives the work and remains visible in the queue.
- Timing. Due and completion times distinguish planned work from finished work.
What to agree during setup
Choose which visit outcomes require a care task and who writes the task text. Agree when an Open task becomes in progress and how an Escalated task is reviewed. Make sure the assigned person is a People record linked to a user. During setup, test a task against a scheduled visit so its patient, assignee and due time are clear. Have the next shift confirm that it can find the task by patient and owner before the due time passes.
Check the handoff on an actual case
A clinician finishes a visit and asks for a call next week. The Care Coordinator records the patient, assignee, task and due time. The clinician opens My Care Tasks, starts the call work and records completion when the call is done. If the patient cannot be reached and the matter requires review, the clinician marks the task Escalated so the Clinical Lead can decide the next step. In the rehearsal, confirm that the patient is correct, the assignee receives the task and the completion time reflects actual work. A second patient with a similar name is a useful check of the patient reference.
