How the model is organised
Identity and access
Patients hold patient identity and assigned clinician; People links staff to Users. Care team assignments records patient-specific clinician access, and clinical privileges records professional standing.
Visits and evidence
Encounters links patient and clinician. Clinical notes, Observations, Specimens, pharmacy dispensations and Charges reference the encounter.
Patient follow-through
Consents, care tasks and Payments reference the patient. Care tasks also names an assignee in People.
Supporting rates
Exchange rates is a read-only effective-rate reference in this app.

Keys and references
Use the patient number for human identity checks and the record references for relationships. An encounter belongs to one patient and names one clinician. The attached clinical evidence belongs to its encounter, while consent, care tasks and payments belong directly to the patient. People connects to Users for account identity; the effective dates on privileges and care-team assignments add timing to those relationships.
Migration order
- Users and People. Establish accounts and staff records before assigning clinicians or task owners.
- Patients. Bring patient numbers and assigned clinicians into the registry, checking duplicates before visits are linked.
- clinical privileges and care team assignments. Enter professional standing and patient-specific access with evidence and effective dates.
- Consents and Encounters. Bring purpose-specific documents and visit history against known patients.
- clinical notes, Observations, Specimens and pharmacy dispensations. Link clinical evidence to the correct encounter.
- care tasks, Charges and Payments. Add follow-up and financial records after their parent patient or encounter is present.
