Where it usually breaks

And how to keep it from happening.

  • A task with no clear patient or assignee can be missed. Read back the patient, action, owner and due date.
  • An escalated task should remain visible to the care team. Do not report it as completed merely because the appointment has ended.
  • A charge and a patient payment are separate records. Match the encounter and patient context during reconciliation.
