How a Community Health Worker App rollout works

Set up the care team, patient records and visit cycle in a practical order.

  1. Agree operating rules

Define patient numbers, visit types, consent purposes, note types and observation units with the clinical team.

  2. Set up people and access

Link people to users, check role access and record care assignments and professional privileges.

  3. Load households

Import a small patient set with contact details, current worker and consent state; inspect duplicate identities.

  4. Rehearse visits

Schedule an encounter, record arrival, observations and a signed note, then review and close it.

  5. Add specialty and finance

Test a specimen, dispensation, charge and household payment when those records are part of the program's scope.

  6. Review handoffs

Check follow-up ownership, due dates and any external lab or accounting connection before wider use.

What the customer provides

Bring the staff directory, patient number policy, household records, consent forms, privilege evidence and a few representative encounters. Name the Clinical Coordinator, Clinical Governance Administrator and Clinical Reviewer who will inspect evidence in the pilot. Provide the specimen and finance handoff rules if those parts are used.

Changing it afterwards

Test a change to visit types, consent purposes or access assignments against existing records before using it broadly. Check the views and reports that depend on the changed values. Keep an earlier signed or verified record understandable when a later correction is made.
