Home visit services
Home visit services rely on clear ownership before a worker travels and on reliable documentation afterward. The household record holds patient identity and a general assigned worker. Each visit separately names the worker scheduled for that encounter. The team can therefore review a change in coverage without losing who handled an earlier episode.

Home visit services rely on clear ownership before a worker travels and on reliable documentation afterward. The household record holds patient identity and a general assigned worker. Each visit separately names the worker scheduled for that encounter. The team can therefore review a change in coverage without losing who handled an earlier episode.
Pilot one routine home visit and one visit that needs follow-up. Set up the households, workers and care assignments before entering appointments. Record arrival and actual work dates, then enter the observations and signed notes. Send the encounter to the reviewer and have the coordinator check the evidence before recording completion.
For the second case, create a household follow-up action with an owner and due date. Have a Licensed Clinician or Clinical Coordinator update its progress and record the completion date. Run a changed-assignee case so the team can decide whether the action should remain with the original person or be reassigned. Use the open-action view during a daily handoff to check attention-needed work.
Agree the unit conventions for observations and the meaning of each visit type. Decide who records a cancellation and who contacts the patient. If the service also bills for visits, the coordinator can maintain visit-linked charges while a household payment record holds the received amount and reference. External invoicing or payment processing should be set up as a separate connection when needed.
