Step by step

1. Prepare household Clinical Coordinator

   Clinical Coordinator confirms the patient number, contact details, assigned worker and current consent state before a visit is entered.

2. Schedule visit Clinical Coordinator

   Clinical Coordinator links the household and worker, enters visit type and planned date, and records the visit as Scheduled.

3. Record arrival Clinical Coordinator or Licensed Clinician

   Clinical Coordinator records Arrived when the worker reaches the household and in progress when care begins.

4. Capture evidence Licensed Clinician

   Licensed Clinician enters measured observations with a unit and recorded-by person, then drafts a note linked to the same visit.

   • Attribute readings and note text

5. Sign documentation Licensed Clinician

   Licensed Clinician reviews the note text, records the signed date and changes its state from Draft to Signed.

6. Review encounter Clinical Reviewer

   Clinical Reviewer reads the visit, note, observations and relevant consent at awaiting review.

   • Decide whether evidence supports closure

7. Close visit Clinical Coordinator

   Clinical Coordinator records Completed after checking the reviewed evidence and checks the actual ended date and any open follow-up owner.

   • Retain completion and next action owner
