Patient intake to appointment review

Carry a patient visit from identity checks to the clinical review handoff.

Step by step

  1. Register patient Clinical Coordinator

Clinical Coordinator matches the patient number to the returning patient and confirms the assigned dentist. For the planned care, check the consent purpose, form version, signature date and expiry. If the patient summary says Active while the relevant consent has expired, bring the discrepancy to Licensed Clinician before admitting the visit.

    • Identity and consent evidence prepared
    • Updates Patients, Consents
  2. Admit appointment Clinical Coordinator

Clinical Coordinator confirms the patient, dentist and scheduled time. Record Status: Scheduled → Arrived → in progress as the patient arrives and care begins, and enter the actual start time. Keep a cancelled visit marked Cancelled so it stays distinct from a completed appointment.

    • Arrival and actual visit progress recorded
    • Updates Appointments
  3. Document appointment Licensed Clinician

Licensed Clinician writes an attributed chart note for the visit and records each measurement with its value, unit and measurement time. When the note is ready, the clinician signs it and records Status: Draft → Signed with the signing date. Agree who may amend a signed note and how the team reviews the amended content.

    • Attributed visit evidence recorded
    • Updates Chart notes, Clinical measurements
  4. Review appointment Clinical Reviewer or Clinical Coordinator

Clinical Coordinator sends the visit to review with Status: In progress → awaiting review. Clinical Reviewer reads the note, measurements and relevant specialty evidence, then communicates the decision to Clinical Coordinator. After that handoff, the coordinator records Completed and the actual end time. Agree how the reviewer communicates and retains the decision during setup.

    • Human review followed by coordinator recording
    • Updates Appointments, Chart notes, Clinical measurements
ApprovalA person approves before the work moves on.

Where it usually breaks

And how to keep it from happening.

  • A returning patient receives another identity because contact information changed.
  • A Draft note is treated as Signed without the signing action or signed at.
  • A read-only reviewer is expected to change Appointments directly.
  • The recorded end time is copied from the schedule instead of the actual visit.
