Where it usually breaks

And how to keep it from happening.

  • A note and observation point to different encounters. The clinician checks both links before signing.
  • An observation has a value without a meaningful unit. The reviewer asks the recorder to correct the evidence.
  • A visit is marked completed before review. The coordinator keeps it awaiting review until the reviewer has inspected the case.
