Hospital emergency care
A hospital emergency care team needs to identify the patient, open the right visit and keep every piece of documented care with that visit. A Clinical Coordinator records arrival and current encounter state. The Licensed Clinician adds notes, observations, specimens and dispensations. The Clinical Reviewer can inspect the evidence before the episode is completed. The patient's earlier visits remain separate from today's care.
How Emergency Department System is set up for you
Arrival to care
The coordinator can move the encounter from Scheduled to Arrived and in progress, record the actual start and see the current distribution on Business Overview.
Clinical evidence
Notes retain their author and signed date. Observations retain the measured value, unit, time and recorder. Specialty records point to the encounter.
Continuing action
A task with a patient, assignee, due date and state carries work into the next shift after the encounter ends.

Pilot a small set of encounters whose patient numbers, assigned clinicians and visit types can be checked against the source record. Include a returning patient so the team can test separate visits under one identity. Record an observation and draft note, then check how they appear to a reviewer. Add one specimen case and one dispensation with its medication lot and quantity. Finish with a care task that stays open after the encounter is Completed.
Agree the exact point when an encounter becomes awaiting review and which person records Completed. Review dated care-team access for anyone who needs the patient record. If clinical, laboratory, pharmacy or billing systems supply other evidence, map each source and handoff before importing it. The team should reconcile source identifiers and sampled records during the first operating cycle.
