Urgent care service
An urgent care service may see the same patient for distinct visits over time. The patient record carries a stable identity and an assigned clinician, while each encounter carries its own visit type, timing, clinician and state. The care team can document the current episode and retain an intelligible history for a return visit. Clinical notes and measured observations stay tied to the encounter that produced them.
How Emergency Department System is set up for you
Patient check-in
The coordinator finds the patient, confirms the patient number and creates a visit. Arrived and in progress show which episodes need care now.
Evidence by visit
The clinician records note type, text and author, then measured observations with unit and time. The reviewer opens the same encounter to inspect them.
Next-day work
A patient-linked care task has a named assignee and due date. Open, in progress and Escalated states make the next action visible.

Choose one week of representative visits and include at least one patient who returns. Reconcile each encounter to its source visit before entering notes and observations. Have the coordinator and clinician test the handoff from arrival to care, then ask the reviewer to identify which evidence belongs to each episode. Add a follow-up task for a patient after the visit closes and verify it remains visible in the open-task view.
Agree how the team records consent for different purposes and how a clinician's professional privilege is verified. Decide who may change encounter status and who reviews overdue tasks. If payment or laboratory records come from another source, agree who enters them and who checks each reference.
