How the model is organised

Patient and staff identity

Patients connects to People through the assigned clinician. People connects to Users for sign-in identity.

Care operations

Encounters holds the patient visit. Clinical notes, Observations, refraction records, lens orders and Charges refer to that encounter.

Consent and follow-up

Consents and care tasks connect directly to Patients; task ownership connects to People.

Governance

Clinical privileges applies to People. Care team assignments connect a patient to a clinician for an effective period.

Money and reference data

Payments connects to Patients. Exchange rates carry read-only currency rates and has no care record reference.

Keys and references

Care records use Id as their internal identity; Users use User and exchange rates uses ID. Patient number is the human-facing patient identifier and accession number is the lab specimen’s matching reference. The patient link on an encounter must resolve before care evidence is added. Notes, observations, specimen records, medication dispensations and charges each use their own encounter reference. A patient payment refers to a patient, while a charge refers to an encounter; any invoice-to-receipt reconciliation is a separate process.

Migration order

  1. Load Users and People, then check clinician user links.
  2. Load Patients with their assigned clinician.
  3. Add clinical privileges and care team assignments with effective dates and evidence.
  4. Add Consents and then Encounters for the patient visits.
  5. Add clinical notes, Observations, refraction records, lens orders and Charges against the correct encounter.
  6. Add care tasks and Payments against the correct patient; check receipt references with the financial source.
