2. Record correction Form Owner

   Form Owner documents why the earlier revision needs a successor.

   • Record the reason against the source.

3. Create successor Operator

   Operator starts the correction action, creating a successor Draft revision with a lineage number and a higher revision.

   • Create the compensating run and numbered steps.

4. Trace operation Operator

   Operator compares the source and successor with the prior and compensating runs.

   • Retain prior evidence and record supersede history.
   • Updates Operation runs, Operation steps, State history

5. Review new revision Independent Reviewer

   Independent Reviewer makes a new decision against the successor source revision. Approval is the human gate before its publication.

   • Keep the earlier decision with the earlier revision.
   • Updates Review decisions, Form definitions
   ApprovalA person approves before the work moves on.
