Have the reviewer inspect the case and ask the authorized person to sign a decision with a reason. Then test a correction: an amended decision and a cancelled action should remain understandable to the next owner. Add more organizations after the team has agreed its review and naming rules.
For the first review meeting, bring the coordinator, a clinician and the person who maintains controls. Have each person locate the same assessment from their own working screen. Compare the linked result and decision, then inspect the organization finding and due action. This shows whether recorded ownership gives the group a clear next action.
