Multidisciplinary home care
A patient may be seen by different clinicians across visits. The patient record carries an assigned clinician, while each encounter names the clinician for that specific visit. Care-team assignments add a patient-specific purpose and effective period. This structure helps a multidisciplinary team read who was assigned to the patient and who participated in a particular encounter.

Start with a patient who has an assigned clinician and more than one planned visit. Add care-team assignments for the clinicians who need patient access. Record consent for the relevant purpose and check its expiry. Schedule visits with explicit clinician references, then create a note and observation on each visit. The review should be able to distinguish those records without relying on a single patient-level note.
Agree how the team changes the assigned clinician and ends an older access assignment. Reviewers can use the current assignment view and encounter history together. The governance administrator maintains People, privileges and assignments, while the coordinator handles patient and visit records.
