How the model is organised

Patient access:

Patients, Appointments and Departments identify the person, clinician, service and planned visit.

Active care:

Encounters anchor clinical notes, Observations, bed assignments and Charges.

Clinical follow-up:

Care tasks and care team assignments attach to Patients; diagnostic orders and medication orders attach to Encounters.

Specialty operations:

Diagnostic results attach to diagnostic orders, while operating bookings attach to Encounters.

Authority:

Consents attach to Patients; clinical privileges attach to People; People link staff to Users.

Collection:

Payments attach to Patients, so settlement against encounter Charges needs an agreed reconciliation process.

Keys and references

Patient number is the operational patient identifier. Each encounter references its patient and clinician. An appointment also references a department. A diagnostic result references its order, which in turn references an encounter. Medication orders reference the encounter for prescribing context. The staff directory references user accounts, while patient care assignments reference staff records.

Migration order

  1. Bring in Users and People, then verify staff identities.
  2. Add Departments, Patients, Consents and clinical privileges.
  3. Add Appointments, Encounters and care team assignments.
  4. Add bed assignments, clinical notes, Observations and care tasks.
  5. Add diagnostic orders, diagnostic results, medication orders and operating bookings.
  6. Add Charges and Payments, then reconcile patient references and service context.
