• Measurement source. Bring observation identifiers, dates, units and values from the system that records the underlying service or outcome. Agree the source field and target field mapping for each goal.
• Document source. Retain the external reference and document date for supporting files so a reviewer can locate the original evidence.
• Delivery provider. Carry provider references and external receipts into the operation trace when an approved action has an external effect.

Start with one source and one program. Compare a source record with its observation line, including unit and outcome value. Keep mapping versions and effective dates so later readings can be interpreted under the right rule. For external actions, retain the operation key, source revision and receipt evidence. When a response is uncertain, verify the provider result against the original reference before another attempt is made.
