Where it usually breaks
And how to keep it from happening.
- A reviewer decides after an operator has changed the program. Compare the decision's source revision with the current program revision before saving it.
- A return has no usable reason. Describe which observation or evidence reference needs correction so the operator can act.
- The reviewer is also the preparer. Keep the review assignment separate from the person who entered the program and evidence.
- A published state is read as evidence that every line was accepted. Check the line outcomes and the approved revision before interpreting the reported value.
