Where it usually breaks
And how to keep it from happening.
- An attachment exists but refers to another program or reporting date. Read its external reference rather than counting the file as verified.
- The follow-up action is marked completed without a resolution. Record what source arrived and how it addressed the question.
- A program is presented for review after the evidence changed but before the revision is checked. Use the current revision and retain earlier review decisions separately.
- A rejected observation is included in an outcome total without a reason. Keep the line's outcome and decision reason visible to the reviewer.
