Platform operations
Platform operations teams often restore a service before the underlying cause is fully understood. The incident record captures the immediate disruption, its assigned responder and recovery time. Response events explain the actions taken. A Problem keeps a recurring issue visible, and a Change gives a planned remedy its own risk, work window and rollback plan.
How Incident Response Platform is set up for you

Action timeline
Keep response events and incident updates attached to the case they describe.
Root-cause work
Give recurring service problems a named owner, status and recorded cause.
Change review
Examine risk, schedule and rollback before approving planned work.
Begin with a service that has recent disruption history. Add its owner, supporting assets and on-call coverage. Recreate one representative incident and ask a responder to record acknowledgment, investigation actions and mitigation. The Service Manager should be able to read the record later and explain what happened, when the team responded and who had the work.
Next, open a Problem for a repeated failure pattern. Write the problem statement around the observed behavior; record a root cause only after the investigation supports it. Draft a Change for the proposed remedy with a clear risk level, start and end, and rollback plan. Let the Change Approver review the plan and the Service Manager record the decision before execution. Verify the result after the work window and revisit the Problem status separately.
A monitoring provider can supply arrival receipts for signals. Agree the mapping to services and the procedure for an event that is rejected or fails handling. The provider record helps explain intake; the incident, problem and change records explain the team's decisions and actions. Review these together at the end of the pilot to see whether the handoffs are clear.
