Outpatient clinic teams
An outpatient clinic may investigate the same case over more than one visit. Each investigation has its own schedule, status, notes and evidence, while the case remains the common identity. The Clinical Coordinator can assign the next episode to an investigator and keep the consent position visible. The Licensed Clinician documents what happened during that episode and gives the next action a due date and owner.

The Clinical Coordinator can assign the next episode to an investigator and keep the consent position visible. The Licensed Clinician documents what happened during that episode and gives the next action a due date and owner.
Start with a returning case and enter a fresh investigation with a new planned date. Confirm that earlier notes remain with the earlier episode. Record a purpose-specific consent and have the coordinator check its expiry date before scheduling. During the visit, add a signed note and an observation with its unit. Create a follow-up action due after the visit, assigned to the person responsible. Ask the reviewer to find the evidence and identify the action that remains open.
Agree who updates status when the person arrives and who moves the episode to awaiting review. The clinic should also agree how an escalated action is surfaced to the coordinator. If an investigation service is charged, the charge record can hold a description, amount and billing status linked to that investigation; payment is recorded under the case. Keep those records separate from the clinical review decision.
