A patient returns for a visit with a different clinician. The governance administrator ends the earlier care team assignment and records the new access purpose and dates. The coordinator links the new encounter to the patient and current clinician. The Licensed Clinician adds an attributed note, and the reviewer checks the new visit’s evidence without changing the prior encounter.
Start with a single service line and a small patient cohort. Define who changes care team assignments and who verifies each privilege before widening access. Use the first operating cycle to confirm who owns each status change and how staff handle an exception. Keep billing separate from care documentation: a charge attaches to the encounter, while a payment belongs to the patient.
