What you can do

Keep the working record clear

A clinical note has an encounter, author, type, body, signed date and state. Draft, signed and amended distinguish preparation from later correction. The reviewer reads the note beside the visit. An observation separately stores metric, numeric value, unit, time, encounter and recorder.

How the work is organized

  • Attribution. The Licensed Clinician links a note to an encounter and an author, and an observation to its recorder.
  • Signing. A draft note remains distinct from Signed and Amended; the signed date is checked with the state.
  • Measurement. A numeric value is read with its metric, unit and observation time.

For a patient whose specimen was received, the clinician records a relevant observation and signs the encounter note. The reviewer compares its author and date with the visit. A correction uses the amended state so the team can distinguish a later change from the initial signing.

What to agree during setup

Agree which note types clinicians use and who signs them. The Licensed Clinician can manage notes and observations; the Clinical Reviewer can read them. Staff should enter the observation time and unit with each measured value so a later reviewer can understand what was recorded. A note’s author and an observation’s recorder are separate references to People.

Decide how the team handles a correction after signing. The Amended state distinguishes a later account, while the record still needs its encounter and author. Rehearse a draft that has no signing date and a signed note with a date before anyone relies on the state alone. Keep the note and measurement on the intended encounter when a patient returns for another visit.

Check the handoff on an actual case

Open two visits for the same patient. On the later encounter, have the clinician enter a note and one observation with a metric, value, unit and observed time. Ask the reviewer to find those records from the encounter and identify the note author and observation recorder. Check that neither record appears as evidence for the earlier visit.

Leave another note in Draft and ask the reviewer whether it can be treated as signed. Then sign the first note and record its date. Walk through an amendment and confirm that the current state remains distinguishable from the original signing. The coordinator can manage the encounter state, while the reviewer reads its evidence before closeout.

What your team sees

  • Clinical Notes to Sign — Draft notes with their encounter and author.
  • Observations by Encounter — Measurements with units and recorder.

Follow the work through

Read encounter and specimen handling for the intake handoff and review and closeout for the final visit decision.
