Where it usually breaks

And how to keep it from happening.

  • The same person completes and verifies the action. Assign an independent reviewer before submission.
  • The action fixes the visible label without addressing its cause. Agree what evidence will demonstrate effectiveness.
  • A parent issue closes while another linked action is still open. Review the entire action set.
  • A failed verification disappears after resubmission. Retain the prior review history when the action returns to implementation.
