Correction to effectiveness

Coordinate containment and corrective owners, then close the failure only after all linked actions are accepted.

Step by step

  1. Contain the failure Quality Manager

Quality Manager follows the containment responsibility established by the inspection decision. Move the nonconformance through Containment and Investigation as the response proceeds. Keep the original inspection link visible during the investigation.

    • Coordinate the immediate response and investigation
    • Updates Nonconformances, Inspections
  2. Plan and implement actions Quality Manager

Quality Manager records the root cause, action, owner and due date for each response. Obtain plan approval from the authority agreed during setup before advancing Approved work to Implementing. Bring completed work to Effectiveness Review with the evidence needed for a decision.

    • Assign and carry out corrective work under agreed plan authority
    • Updates Corrective actions, People
  3. Review effectiveness Quality Approver

Quality Approver evaluates the exact action and linked failure. An accepted decision closes the eligible action. An unsuccessful decision remains in the attributed review history and does not earn closure. The reviewer must act against the current version of the work.

    • Retain the reviewer decision and close accepted action scope
    • Updates Corrective actions, Corrective action reviews, Nonconformances
ApprovalA person approves before the work moves on.
  4. Confirm the failure outcome Quality Manager

Quality Manager checks the complete set of linked actions. The review process closes the nonconformance only when every linked action has been accepted. This final state follows the accepted action history; it is not an extra manual approval of the same decision.

    • Check closure after all linked actions are accepted
    • Updates Nonconformances, Corrective actions

Where it usually breaks

And how to keep it from happening.

  • Early closure. A completed repair can distract from another action still awaiting review. Examine every linked action.
  • Completion without effectiveness. The owner finishing the task does not replace the reviewer's evidence decision.
  • Repeated decisions. A retry must preserve the accepted or rejected operation history. Changed input cannot reuse the earlier operation identity to rewrite it.
