Multidisciplinary care teams
When several clinicians take part in a patient's care, the team needs to know who is assigned, which encounter holds the evidence and who owns the next task. The patient record carries an assigned clinician. Care-team assignments add a purpose and effective period. Individual notes and observations retain their own author or recorder, so the visit remains understandable after a shift change.
How Medical Coding System is set up for you
Care-team access
An Active assignment names a patient and clinician for a stated purpose. Ended records show that the arrangement no longer applies.
Attributed evidence
A note belongs to its author and encounter; an observation names its recorder, measurement, unit and time.
Follow-up work
A care task names the patient, assigned person and due date. Open, in progress, Escalated and Completed show the next worker what happened.

Start with a patient who has a primary clinician and another clinician involved for a specific purpose. The governance administrator records each relevant privilege and assignment. The coordinator schedules an encounter, and the clinicians record their own evidence. A reviewer examines the visit and checks who made each entry. After the visit, assign a follow-up task to one person with a clear due date.
Rehearse the task becoming Escalated, then returning to active work before completion. End a care-team assignment separately from the task. These different states keep access history and work completion from becoming the same signal. If the team changes staffing, confirm the patient's assigned clinician and each open task owner before the next appointment. The coordinator can then prepare any service charge from the encounter without guessing who provided the care.
