What you can do
Know who is entitled to do the work
The Clinical Governance Administrator maintains clinician privilege records. Each record names a clinician, privilege type, license number, issuing authority, verification evidence, effective dates and current status. Pending, Verified, Suspended and Expired distinguish evidence that is being checked from evidence the organization considers current. The dates matter because an earlier verified privilege may no longer cover a later visit.
Care-team assignments connect a clinician to a patient for a stated access purpose and effective period. Active and Ended describe the assignment’s current state. These records give the team a way to review who is associated with a patient and why. The patient and clinician references should be checked before access or work is granted for a new care episode.
How the work is organized
- Privilege evidence. The administrator keeps verification documents and effective dates with the clinician.
- Care-team assignment. The patient, clinician and purpose remain together for access review.
- Follow-up ownership. Care tasks name a patient, assigned person, due date and status.
What to agree during setup
Agree who verifies privilege evidence and how often effective dates are reviewed. Decide what an Active care-team assignment permits in operating practice and when it should be Ended. The Clinical Governance Administrator manages the privilege and assignment records; the Clinical Reviewer can inspect them during care evidence review. The Licensed Clinician records assigned care tasks and can see the relevant privilege and assignment context.
A follow-up task should describe a concrete action. Open, in progress, Completed, Escalated and Cancelled distinguish ongoing work from finished or redirected work. The team should decide who receives escalated tasks and how an overdue due date is handled. A task assigned to an inactive person needs attention before it becomes invisible in someone’s personal queue.
Check the handoff on an actual case
Before a clinician handles a new patient, the administrator checks the relevant privilege type and effective dates, then verifies that a care-team assignment names the same patient and clinician for the intended purpose. The reviewer can inspect those records during encounter review. If the privilege is Suspended or Expired, the team should resolve that condition through its authorization policy before relying on the earlier verification.
After the visit, the clinician assigns a follow-up task with a due date and named person. The next worker can see whether it is Open or in progress. If it needs attention outside the expected path, Escalated makes that state visible. When finished, completed at and Completed document the close of the task. The clinical note and the task remain separate: one records care, the other assigns future work.
