How the model is organised

People and patients

Users and People represent staff access and identity; Patients holds the person receiving care.

Care episodes

Encounters connects a patient and clinician to dated work. Clinical notes, Observations, medication administrations and resident care plans connect directly to Encounters. The last record stores specimen collection and accession information with lab-style statuses.

Care coordination

Consents and care tasks connect to Patients. Care team assignments connects a patient and clinician, while clinical privileges connects to a clinician.

Financial records

Charges connects to Encounters; Payments connects to Patients. Exchange rates holds read-only effective rates.

Keys and references

The encounter is the parent for episode-specific evidence and charges. The patient is the parent for consent, task and payment records. Staff references point to People, which links to Users. The patient assignment and encounter clinician are separate references. A payment is patient-linked; use its receipt reference when reconciling with the external financial source.

Migration order

  1. Staff access. Prepare Users and People, then clinician privilege information.
  2. Patient identity. Import Patients and confirm assigned clinicians.
  3. Care access. Add Consents and care team assignments with their dates and documents.
  4. Episodes. Import Encounters before dependent notes, observations, medication and specimen records.
  5. Follow-up and finance. Add care tasks, Charges and Payments after their patient or encounter parents exist.
