Residential care teams
A residential care team can use the patient record as a shared identity and the encounter as the episode being worked. The coordinator sees scheduled and review-ready episodes. Clinicians record notes, observations and medication administration against the appropriate encounter. The reviewer examines those records with the episode before it is completed.
How Nursing Home / Assisted Living is set up for you
Medication visit
A clinician records the medication, quantity, unit, lot and authorizing person for the encounter, then adds a note or observation when the work calls for it.
Follow-up after care
The team creates a patient task with an assignee and due date, then tracks completion or escalation independently of the earlier encounter.
Consent check
A purpose-specific consent record shows the signed version, dates and document for the patient involved in the episode.

Start with one type of recurring care episode and a small group of patients and clinicians. Agree on visit type naming, note types, observation units and the point at which the coordinator sends an encounter to review. Rehearse a medication entry for one encounter and a follow-up task for the same patient. The care team should verify that the authorizing person and assigned clinician references are correct before relying on those lists in daily work.
Use the review queue to discuss incomplete evidence while the episode is still open. If a patient has more than one encounter, ask the reviewer to trace each note and medication record to its correct episode. Give the coordinator a clear routine for escalated tasks and signed consent checks. A pilot is ready when staff can move an episode from schedule to review, locate its evidence and identify the next accountable person.
