What you can do

Place each entry with the right episode

Licensed Clinician records medication administration against an encounter. The entry identifies the medication, lot number, quantity, unit and authorizing person. A reviewer can find the entry through its encounter rather than reconstructing which patient visit it belonged to. This is useful for a residential care team that records medication work alongside clinical notes and observations for the same episode.

The authorizing person is a staff reference. The team should confirm that it points to the person who authorized the recorded administration. The record is strongest when the medication name, quantity and unit are entered consistently and the lot is captured when staff need that trace. Because the entry has an encounter reference, the coordinator can review the episode and its supporting medication record together.

How the work is organized

  • Episode link. The encounter identifies the patient visit or care episode associated with the medication entry.
  • Administration detail. Medication, quantity, unit and lot describe what the clinician recorded.
  • Authorization reference. The authorizing person is named on the record for later examination by the reviewer.

What to agree during setup

Decide which care episodes call for a medication entry and which staff will enter it. Agree on a consistent medication naming convention and unit vocabulary before a pilot; free-text variation makes later review harder. Confirm the team’s rule for lot recording. The clinical team should also decide how it verifies that the authorizing person is the right staff member for the entry, using the privilege and care assignment records where applicable.

Set a review point between documentation and encounter completion. A medication record should be examined with the associated note or observation when those records describe the same work. Keep corrections visible through your normal record review procedure and make sure staff use the proper encounter when a patient has more than one episode on the same date.

Check the handoff on an actual case

A patient has a scheduled medication visit. The coordinator opens the existing patient record and selects the clinician. At the visit, the clinician records the medication name, quantity and unit, notes its lot and chooses the authorizing person. They check that the entry is attached to the current encounter. The reviewer then sees the medication entry in the episode’s evidence context, alongside any clinical note or measured observation. The coordinator completes the encounter after the evidence review.

What your team sees

  • Medication record by encounter — Examine the medication, quantity, unit, lot and authorizing person for each episode.
  • Encounter review queue — Find episodes waiting for an evidence review before completion.
