How the model is organised
Patient foundation
Patients hold identity and assigned clinician; People represents staff and connects to Users. Care team assignments records patient-clinician access periods.
Visit activity
Encounters holds the scheduled visit, care timing, clinician and status. Clinical notes, Observations, Specimens, pharmacy dispensations and Charges refer to the encounter.
Patient activity
Consents, care tasks and Payments refer to Patients. Clinical privileges refer to People.
Reference rates
Exchange rates is a read-only effective-rate record available to the app.

Keys and references
The patient number supports identity checks across visits. Encounter Id is the reference for visit-specific care evidence. Patient-linked tasks, consent and payments remain accessible across encounters. People connects clinical assignments, authorship, observation recording and dispensation authorization to staff. Preserve these references when importing data; a similar display name is not a substitute for the correct patient or clinician link.
Migration order
- Staff and access. Confirm Users and People before assigning clinicians.
- Patient identity. Import Patients and reconcile patient numbers.
- Governance. Add clinical privileges and care team assignments with their dates and states.
- Visits and consent. Add Encounters and Consents after patient references are stable.
- Care evidence. Load clinical notes, Observations, Specimens and pharmacy dispensations against the correct encounters.
- Follow-up and finance. Add care tasks, Charges and Payments, checking their distinct patient and encounter links.
