Intake and consent

Identify the patient, assign care ownership and record purpose-specific consent.

Step by step

  1. Find the patient Clinical Coordinator

The Clinical Coordinator checks the patient number and name before creating a new record. A duplicate identity would split encounters and follow-up work.

    • A stable identity and contact record
    • Updates Patients
  2. Assign the clinician Clinical Coordinator

The coordinator names the clinician responsible for the patient. Governance staff confirm the care-team assignment and its effective period.

    • Named care ownership with dates and purpose
    • Updates Patients, Care team assignments, People
  3. Record consent Licensed Clinician

The Licensed Clinician records the consent purpose, version and document. A pending consent stays visible until it is signed.

    • Signed purpose and version attached to the patient
    • Updates Consents, Patients
  4. Review current state Clinical Coordinator

Staff check the patient-level consent status together with the specific consent record before planning the visit. A withdrawn or expired consent needs attention for its purpose.

    • Current consent state checked before care
    • Updates Patients, Consents
