Who it's for

Multispecialty care teams

A patient can be referred to more than one specialty. Each request has its own source, receipt date, priority, status and follow-up records. A multispecialty team can use those distinctions to avoid closing one request when another still needs review. The patient record gives the team a common identity, while assignments identify the clinicians connected to that patient for a stated purpose and period.

How Patient Referral Management is set up for you

Separate requests

Keep each specialty referral as its own record. Check the specialty and referring provider when several requests share a patient.

Route review

Use the received queue and its board to inspect requests awaiting review. The reviewer records the decision for the particular referral being considered.

Check coverage

Compare the clinician's active assignment, access purpose and verified privilege with the action and date at hand.

Coordinate care

Use patient care tasks for work across the team, and referral follow-ups for outcomes belonging to a specific specialty request.

Choose a patient with two specialty requests and assign different clinicians where appropriate. Record a current assignment and privilege for each clinician, then create an encounter for the patient whose request reaches scheduling. Staff should confirm which request the visit concerns. Ask the coordinator and reviewer to identify which follow-up belongs to which referral. A second rehearsal can include an Ended assignment or Expired privilege so the team knows who should resolve the gap before continuing.

Agree on specialty wording before adding a larger intake stream. Review who can change patient identity, who can progress referral statuses and who owns an escalated care task. External provider communication can be connected after the internal status and responsibility handoffs are clear.
