What you can do
Therapy plans in daily work
A therapist drafts a plan under the care request. Patient reference, planned sessions and therapist give the reviewer a concrete scope to inspect. The coordinator submits the plan only after checking its parent request and current revision.
The plan moves from Draft to Submitted for clinical review. The reviewer records a decision for the same request and revision, then the plan can become Approved or Rejected. A completed plan stays linked to the originating request.
How the work is organized
- Source. The therapist prepares a plan linked to the request. Care Coordinator checks its patient reference, planned sessions, therapist and current parent revision before submitting.
- Decision. Authorized Clinical Reviewer records a reasoned decision for that parent and revision. An older decision cannot approve a changed plan.
- Follow-up. A Return decision calls for corrected material and another review of the current revision. Preserve the earlier reason so the next reviewer sees what changed.
What to agree during setup
Agree who writes the plan, who submits it and what supporting clinical evidence the reviewer must see. Set a rule for changes after submission so the approved revision remains identifiable.
Confirm which team member can act for the practice and how staff check the current revision. Rehearse a changed case with the person who receives this handoff.
Check the handoff on an actual case
Take a plan whose session count changes during review. Ask the reviewer to compare the patient reference, therapist and session count with the supporting material. Test that a decision for an earlier revision cannot approve the changed plan.
Have the next person open the linked request and explain what changed, who decided, and which evidence supports the current state. Repeat with a stale revision; the team should return to the current record before acting.
A useful review case includes a plan returned for more information. The therapist can correct the plan and the coordinator can submit the current revision again. Keep the earlier decision and its reason attached to the request, so the reviewer can see what changed between submissions.
The plan belongs to both its parent care request and its practice organization. That link helps the reviewer compare the planned sessions with the same patient reference and source revision considered for authorization. A completed plan remains part of the request history alongside the decision and supporting evidence.
