What you can do
Open the issue from a reviewed failure
When a reviewer rejects failed inspection scope, the disposition workflow creates a linked Nonconformance for containment. The issue records detection date, severity, description and status while retaining its reference to the inspection. The reviewer can return the inspection and issue links together, so the quality manager has a direct path from the decision to the affected work.
Open Nonconformances shows issues still moving through containment, investigation and disposition. A quality manager uses that queue to find what needs action and to make sure an issue is not closed before the related corrective work is accepted. The inspection reference keeps the original check results and observations within reach.
How the work is organized
- Create the link. Reviewer makes the inspection decision and opens containment work for a failed scope.
- Investigate the issue. Quality Manager records the severity and description, then follows the issue through containment, investigation and disposition.
- Connect corrective work. Corrective Action Owner records the action against the same Nonconformance; Quality Manager checks every linked action before closing the issue.
The issue and its corrective actions have separate states. A nonconformance can be in disposition while an action is still under effectiveness review. That split helps the owner see why the quality issue remains open after implementation work has been submitted.
What to agree during setup
Agree how to describe the affected scope from an inspection rejection. Use the inspection subject reference, linked check results and any lot evidence to make the issue clear. Decide who owns initial containment and who is responsible for changing the issue state. Set a convention for severity so a major or critical issue receives the right attention.
Where a physical hold or production action is necessary, arrange that work with the operating team. The quality record captures containment status and the linked decision. Your local operating procedure should state how the affected material or work is controlled while the investigation proceeds.
Check the handoff on an actual case
A reviewer finds a failed check in a final inspection. The reviewer rejects the inspected scope, and the workflow creates a linked Nonconformance. The quality manager opens the issue, reads the inspection evidence and records the affected scope in its description. After immediate containment, the manager moves the issue into investigation and asks a Corrective Action Owner to record root cause and action.
Before closing the issue, find the linked corrective action and confirm its effectiveness decision. Try to close while an action is still under review; the issue should remain open. Then complete the accepted action path and close the issue from disposition. The linked inspection should still show the rejected outcome that started the case.
