What you can do
Preserve the decision context
An assessment can carry review decisions, attachments and operation receipts. Each serves a different purpose. A review decision names the reviewer, the assessment revision, the decision and reason. An attachment stores supporting material with its uploader and visibility. An operation receipt records an action, expected revision, result state and references to its effects. Together these records let the team reconstruct why a package moved forward or was held.
Imagine an assessment whose test evidence is complete but whose return approval operation reports a failure. The reviewer should not infer that a failed operation changed the package. The receipt supplies the operation key and failure reason for investigation. The team can inspect the assessment and actual effects before deciding what to do next.
How the work is organized
- Review decision. The named reviewer records an approval, rejection or return against a source revision, with a reason.
- Evidence attachment. A title, type, uploader and visibility help the team understand and handle each file.
- Operation receipt. A stable operation key, result state and effect references support exception review.
What to agree during setup
Agree which materials should be attached to the assessment and what may be marked shared. Define an evidence naming practice that lets a reviewer find the relevant sample without opening every file. Assign who can make an independent decision and who investigates a failed or needs-review operation receipt. The person investigating should compare the receipt with the records actually present before treating it as completed.
Record a reason for every return or rejection. The reason should tell the Control Owner what must change. Keep decisions tied to the assessment revision; otherwise a later scope edit may make an old approval look current. The same care applies when a reviewer is asked to approve a regulatory return using the parent’s evidence.
Check the handoff on an actual case
Attach a test sample to an assessment and choose its visibility deliberately. Ask a second person to identify the uploader, type and related assessment. Have the Independent Assurance Reviewer record a decision with a reason against the current revision. Change the assessment revision and check that the earlier decision still points to the version the reviewer saw.
Create a receipt for a guarded operation and inspect both a completed and a failed result. For the failed case, follow the operation key and effect references to the actual records before making a new decision. This rehearsal tells the team whether an exception can be reconciled without repeating an uncertain action.
What your team sees
- Failed operations — Investigate failed and needs-review receipts with their keys and reasons.
- Assessments in review — Read the assessment and supporting decisions at the review handoff.
- Returns awaiting decision — Follow the parent evidence before deciding on a package.
Follow the work through
The assessment review process records the decision. Use completion review to investigate operation receipts before closing the reporting cycle.
