Post-discharge monitoring

A post-discharge program often has a short period in which the team needs frequent readings and clear ownership. The patient record connects consent and assigned clinician to the enrolled device. Each reading points through that enrollment to the patient; each follow-up task has an assignee and due date. The team decides what requires a new encounter and what can be handled through the recorded follow-up process.

How Remote Patient Monitoring is set up for you

Establish the handoff

Record the patient number, contact phone and current signed consent before monitoring begins.

Pair the device

Use the device identifier and metric type to keep incoming values with the correct patient.

Act on quality

Route questionable or missing readings for review rather than treating the flag as a clinical conclusion.

Document the response

Use care tasks for assigned actions and encounters for visits with observations and a signed note.

Choose a representative discharged patient with one monitoring device and a named clinician. Capture the consent document and care team assignment, then rehearse a device enrollment and a late-arriving reading. Assign a follow-up task, move it through progress and have the reviewer confirm closure. Schedule an encounter when the clinical team decides a visit is appropriate. Record its arrival only after identity and consent checks; complete it after the note and tasks are resolved.

Also test a patient whose consent is withdrawn during monitoring. The coordinator should be able to identify the changed state before the next visit. Review earlier encounters and readings with authorized staff while applying the current consent decision to new actions. Review the result with the care team before relying on the process for a larger cohort.
