What you can do

Keep a measurement tied to its source

A remote reading has an observed time, value, unit, quality flag, device enrollment and reviewing person. The enrollment leads back to the patient and metric type. These links matter when a clinician decides whether a number belongs in the patient’s care review. A transmitted value without its unit or enrollment can be misleading, so the capture step resolves the linked records before the team acts on it.

The reading capture step receives the selected enrollment, observed time, value, unit, quality flag, reviewer and source evidence. It checks the patient, actor, reviewer, current verified privileges, care team assignments, encounters and existing readings. The quality flag distinguishes valid, questionable and missing readings. It describes the quality of the captured measurement; it does not itself state a diagnosis or resolve an encounter.

How the work is organized

  • Capture. Device Coordinator submits the reading with time, value, unit and source evidence.
  • Context. The enrollment connects the measurement to the device and patient.
  • Review. Monitoring Clinician uses the quality flag and patient context to decide follow-up.

What to agree during setup

Define how the sending system identifies an event, the accepted units for each metric and who handles a questionable or missing transmission. Decide how the feed maps its device identifier to an active enrollment. Establish the review cadence and escalation route with clinical leadership. A quality flag is a useful work queue, but it cannot replace a clinician’s judgment. Keep the source connection, threshold policy and notification path visible in the operating procedure.

Test late and repeated readings. An observation may arrive after an encounter has been scheduled or after the device has been replaced. The team should still identify its original enrollment and observed time. Compare a repeated source event with the existing reading before treating it as a new measurement. If the source sends a corrected value, check which recorded reading the team reviewed.

Check the handoff on an actual case

A patient sends a value from an enrolled device. Device Coordinator confirms the device association and the reported unit. The reading is marked questionable because the source indicates uncertain quality. Monitoring Clinician checks the enrollment, patient, observed time and value, then decides whether a care task or encounter is needed. The reviewer remains attributable. The team compares a second copy of the transmission with the earlier reading before assigning fresh clinical work.

What your team sees

  • Questionable readings — Work through measurements that need a quality review.
  • Active monitoring patients — Confirm the patient and assigned clinician behind an enrollment.
