Start with one assessment that has a known document and a dated test line. Ask the reviewer to return it for a specific correction, then have the owner revise and resubmit the same parent case. A second decision can reference the new revision while the earlier return remains visible. Inspect both decisions as an Auditor to confirm the authorized read path and the reason trail.
The test record supports a concrete question: when was this evidence checked, what type was it, and was it marked verified? The reviewer can read that line with the attachment rather than relying on a summary in the assessment subject. For an application, the team can see the applicant, submission time, provider application reference and decision date under the related assessment. The team should agree the handoff to its external registry or provider during setup.
