Who it's for

Software delivery teams

Software delivery teams need to see whether an unresolved issue threatens a project commitment. The risk register records the forward concern; the escalation holds the current defect or change item. A project manager can bring both into a dated review without merging their different meanings.

How Risk Management is set up for you

Technical risk

Record likelihood, impact, mitigation and owner against the affected project.

Issue triage

Qualify an Open escalation by severity and retain the triage receipt.

Delivery response

Assign investigation or mitigation work through the project's work packages and tasks.

Review checkpoint

Set a due date for the decision about remaining exposure.

Software delivery teams need to see whether an unresolved issue threatens a project commitment. The risk register records the forward concern; the escalation holds the current defect or change item. A project manager can bring both into a dated review without merging their different meanings.

Start with one active project and a small set of concerns that the team already discusses. Use a Technical risk whose mitigation depends on a task assigned to a developer. Create a High severity escalation for a current defect. The Delivery Lead should triage the issue, then the Project Manager should decide whether the defect raises the risk's likelihood or affects the project date.

The task and escalation can advance at different speeds. A task may be in review while the issue remains in progress; a reported fix can be Fixed before the Evidence Reviewer verifies it. At the project review, the Risk Owner states whether the mitigation changed exposure. This preserves the difference between finishing work and accepting the risk response.

Agree which team member owns each item, how severity is chosen and when a Fixed issue is checked. Keep the due date on the review checkpoint close enough to support a real decision. If the review is missed, record that outcome and set the next action from the project record. A supporting file on the project can aid verification; check its parent record before citing it.
