Plant safety teams
A plant safety team needs the event account, control test and corrective response to tell the same story. The investigation captures when and where the incident or near miss happened. The assessment defines the period and operating unit. A risk describes the hazard; an active control has an owner and effective date. The tester's sample and exceptions show what was observed.
How Safety / Incident Reporting is set up for you
Event context
Investigator records the site, occurrence and severity rank while the details are available.
Control check
Control Tester ties the sample result to a named control and assessment.
Response owner
Action Owner records the correction and evidence by its due date.
Independent result
Independent Reviewer retests the finding and records the decision reason.
A plant safety team needs the event account, control test and corrective response to tell the same story. The investigation captures when and where the incident or near miss happened. The assessment defines the period and operating unit. A risk describes the hazard; an active control has an owner and effective date. The tester's sample and exceptions show what was observed.
Consider a loading area where a near miss suggests that the current separation control was missed. The event record identifies the location, while the failed test and finding describe the gap. The action owner can record a revised measure or practice and submit evidence of completion. A retest checks whether that response addresses the original finding. The Safety Manager can see the current work and its reviewer outcome without collapsing those different decisions into one status.
Choose one plant operating unit and a short assessment period. Enter the control and its owner, agree the likelihood and impact scale, and collect a small representative sample. Use one known exception to rehearse a finding and action. Ask the reviewer to explain the evidence required for Pass, Fail and Inconclusive retest results before closing the pilot case.
Set attachment titles that identify the assessment and event. Agree when evidence should be Shared and who may upload it. Check that the final investigation status follows a recorded decision and that any failed or uncertain operation receipt is investigated.
