Step by step

1. Record investigation Investigator

   Investigator captures the site, occurrence, severity rank and corrective action context against the relevant assessment and organization. Keep the account in Draft while its facts and evidence are being checked.

   • Set incident context and parent
   • Updates Safety investigations, Assessments

2. Submit account Investigator

   Investigator moves the case to Submitted once the incident details and supporting material are ready. The revision identifies the version being presented to the Independent Reviewer.

   • Present the revision and evidence
   • Updates Safety investigations, Record attachments

3. Inspect evidence Independent Reviewer

   Independent Reviewer reads the parent assessment, related finding, action and attachments. Compare the recorded site and occurrence with the supporting evidence before deciding.

   • Check response and independent result
   • Updates Findings, Corrective actions, Retests

4. Review decision Independent Reviewer

   Independent Reviewer records Approve, Reject or Return, the reason, date and source revision under the assessment. Match that revision to the submitted investigation. Approval belongs to the reviewer after checking the submitted facts and retest.

   ApprovalA person approves before the work moves on.

5. Complete case Safety Manager

   Safety Manager checks the persisted decision and verified response before the investigation reaches Completed. A failed or uncertain operation receipt stays visible for review.

   • Updates Operation receipts, Safety investigations

Investigator

Prepares the record; Independent Reviewer decides; Safety Manager checks completion.
