Exception reconciliation

An operation receipt captures a guarded action attempt with an operation key, expected revision, result and time. When the result is Failed or needs review, Inventory Reviewer needs to inspect the source request and any resulting movement before the team repeats the action. The receipt is the starting point for investigation, not a stock balance by itself.
