Capture a Case and Review Evidence

Establish the actual incident scope and present source-identified evidence for independent review.

Step by step

  1. Capture the report Utility Coordinator

Utility Coordinator records the actual reported time, affected units and external incident identity. Select the intended organization and service area and identify the case owner. Keep the asset reference when it helps the team locate the operating context.

    • Establish Reported case with actual scope and ownership
    • Updates Service cases, Service areas, Organizations, Team members
  2. Triage and assign Utility Coordinator

The eligible triage action verifies the external identity and assigns an authorized responder. Follow the case through Reported, Triaged and Assigned as the handoff progresses. Retain the stable operation identity and receipt so the team can inspect the result of the request.

    • Verify incident identity and assign eligible work
    • Updates Service cases, Team members, Operation receipts
  3. Prepare source context Utility Coordinator

Utility Coordinator locates the source evidence and prepares supporting files through authorized actions. Read the measurement with its unit, occurrence time and source reference. Keep the relevant revision clear before asking the supervisor to decide.

    • Locate identified evidence and prepare the review handoff
    • Updates Service evidence, Record attachments, Service cases
  4. Review independently Authorized Utility Supervisor

Authorized Utility Supervisor checks the identified source revision through the guarded evidence-review action. Organization and actor scope must be valid, and the reviewer remains independent of the preparer. Record approval, rejection or return with a reason that explains the evidence decision.

    • Record an independent decision against the intended revision
    • Updates Service evidence, Review decisions, Operation receipts, Service cases
ApprovalA person approves before the work moves on.
  5. Follow the outcome Utility Coordinator or Auditor

Utility Coordinator reads the review reason and action result. Resolve a returned question before presenting the relevant work again. Auditor can trace the source revision and operation identity without changing the records or executing the action.

    • Explain the decision and any required follow-up
    • Updates Review decisions, Operation receipts

Where it usually breaks

And how to keep it from happening.

  • A similar service-area name leads to the wrong organization scope. Check the actual code and references before routing work.
  • A measurement is read without its unit or source. Resolve the observation's meaning before reviewing it.
  • The request uses an older revision after scope changes. Present the current work for the guarded action.
  • A read-only evidence view is treated as editing permission. Use the authorized source or workflow path for changes.
