Corrective action verification

Implement owned corrective work and require an independent effectiveness outcome before closure.

Step by step

  1. Contain nonconformance Quality Reviewer

The Quality Reviewer coordinates the issue through the agreed containment and investigation procedure. Read the failed inspection evidence and retain a description that explains what needs to be addressed before disposition.

    • Record the issue's containment and investigation context
    • Updates Nonconformances, Inspections, Inspection checks
  2. Implement corrective action Action Owner

Action Owner records the root cause, action plan and due date. Follow the agreed plan approval procedure, then carry out the work and submit implementation evidence for effectiveness review. Keep the parent nonconformance link clear when several actions address the same issue.

    • Prepare owned work and submit effectiveness evidence
    • Updates Corrective actions, Nonconformances, People
  3. Verify effectiveness Quality Reviewer

An independent Quality Reviewer examines the submitted outcome. Accept supported work before closing the action. Failed verification returns the work to implementation with the earlier context retained, so the owner knows what needs further attention.

    • Accept and close or return the action to implementation
    • Updates Corrective actions, Nonconformances
ApprovalA person approves before the work moves on.
  4. Check parent closure Quality Reviewer

The Quality Reviewer inspects the outcomes of every linked corrective action. The parent nonconformance can close only when all have accepted outcomes. An individual closed action must not hide other linked work still awaiting verification.

    • Close the parent only after all linked outcomes are accepted
    • Updates Nonconformances, Corrective actions

Where it usually breaks

And how to keep it from happening.

  • An action plan gives no useful explanation of the root cause. Clarify the intended change before implementation.
  • The person who performed the action also attempts to verify it. Use the independent review arrangement configured for the laboratory.
  • A parent issue is closed while another linked action remains unresolved. Inspect the full action set before closure.
